Home/Hospital Guidelines

Eligibility:

Students with acute or chronic health conditions or injuries that significantly limit their ability to attend school may qualify for the Home/Hospital Instruction Program. This program requires a recommendation from a licensed medical doctor, who determines that the student is medically unable to attend school for a period of no less than two weeks. Home/Hospital Instruction serves as a temporary educational setting, available for up to six weeks. Extending this service beyond six weeks necessitates a renewed Physician’s Verification form to confirm the ongoing medical need for home instruction. This form must be submitted to the school before the initial six-week period expires if the student requires further instruction. It's crucial to understand that the final decision on eligibility and approval for home instruction is not made by the medical doctor but by the school team, which may include administrators, special education program specialists, or a multidisciplinary team designated to assess each case.

Service Option:

A student who is determined eligible for Home/Hospital Instruction may receive 300 minutes of instruction per week by a credentialed teacher (one hour per day per week). It is the responsibility of the parent/guardian to be present in the home at the time the teacher is there, and to provide a quiet, appropriate learning environment for study at that time period each day. 

How do I request for H/H instruction for my child?

  1. Contact your school team (e.g., your child's teacher, school nurse, or the special education office) to express your interest in Home/Hospital instruction.
  2. Complete the "Authorization for Disclosure of Medical and/or Educational Information" form. This form allows the school to obtain and review medical and educational information necessary to consider your request.
  3. Fill out the "Request for Home/Hospital Instruction" form, ensuring to include the physician’s detailed statement regarding your child's medical condition and need for home-based instruction. Present this form to the school administrator.
  4. Submit all completed forms to your designated school team. They will review your request and make a final determination on eligibility for Home/Hospital instruction.
